Details and Requirements: Immediate opening for a full-time Community Health Worker, focused on diabetes management. A Community Health Worker (CHW) builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. The primary role of the Diabetes Management CHW is to provide information and support to patients with diabetes who are using G2L’s mobile phone app as part of their health care plan. This is an entry-level role paying $19/hr with excellent benefits, based out of SeaTac. Who We’re Looking For: Experience in outreach, community engagement, health coaching, or related; Experience using smart phone applications. Ability to become an expert in the G2L app and teach others how to use it. **Ability to successfully complete all required training to become a certified CHW. This includes attending an in-person training in Tacoma on July 12th and August 30th.** Preference may be given to candidates who: Have already completed the Washington State Department of Health CHW training; Have experience with health coaching for chronic disease management or other experience with health care.
